The Perfect Roster

Everyone should have one master "Team Binder" and one copy. We want to provide you with the correct information required to make your "Team Binder". Our goal, is for everyone to have a ZERO mistake roster at the Association Championships, this is a simple process that will help keep your team organized.

Items required before your Championship Roster can be signed by your local J O Commissioner:
	- Fully completed "Official Championship Roster" - Proof of age document for each player - Recent photo for each player - Signature for each player – 2004 ASA ID card for each player – Court ordered name change documents if applicable
Roster checklist:
	- "Team Manager's Affidavit" section must be completely filled out – Each players section must be completely filled out - "Division & Classification of Play" is GFP-B/C <??> and Under - Players name must match their proof of age document
Team Binder:
	Items needed:
	- A simple three ring binder - Plastic page sleeves available at any office supply store
	Binder make-up:
	- Place the original roster as the first page - Place the birth certificate, photo, signature and ID card on a single page - Put everything in the name order of the roster

note: Your binder will be turned in to the tournament director at each Championship event. Keep an exact copy of your binder for sanctioned events. Do not give your original documents to anyone other than a Championship event director.
